Lipoma, removal of, by surgical excision or liposuction, if:(a) the lesion:(i) is subcutaneous and 150mm or more in diameter; or(ii) is submuscular, intramuscular or involves dissection of a named nerve or vessel and is 50 mm or more in diameter; and(b) a specimen of the excised lipoma is sent for histological confirmation of diagnosis (H) (Anaes.)
